Ireland ended a closely-fought first quarter with a 19-14 lead. Italy drew level twice before Charlie Molony's powerful 20th-minute finish rewarded a clever attack.
Earlier efforts from captain Josh Neill and Duinn Maguire had been cancelled out by Enoch Opoku Gyamfi and Erik Meroi, but Andrew Browne's side were good value for a 22-14 half-time advantage.
Daniel Green split the posts with a 37th-minute penalty to leave eight points in it, as the Irish scramble defence and counter-rucking prevented Italy from responding before the interval.
Determined to follow up on victories over the USA and Fiji, Ireland gave themselves a match-winning cushion when Charlie O'Shea and Seán Walsh - with a well-taken intercept try - both crossed during the final 10 minutes.
Italy had made light of Jaheim Wilson's sin-binning for a high tackle, scoring through Luca de Novellis to make it 22-21 with just under a quarter of an hour remaining.
However, O'Shea and Walsh showed their finishing skills to ensure Browne's youngsters won the 9th place play-off, even if Italian replacement Nikolaj Varotto claimed the game's ninth and final try.
This was the teams' third meeting of the season, following Ireland's 29-10 victory in January's challenge match in UCD, and their hard-fought 30-27 success during the Six Nations in February.
O'Shea and Rob Carney returned to Ireland's starting XV, while head coach Browne switched Green to full-back and UCD clubman Molony to the right wing.
Influential flanker Neill, the side's third different captain of the Junior World Championship, led a pack which was unchanged from Monday's tough six-try battle with Fiji.
Despite Italy winning an initial scrum penalty, Donnacha McGuire forced a knock-on in a tackle, and Alex Lautsou was central to a spritely counter attack straight through the middle.
A controlled bout of attacking took them closer, and after hooker Maguire was stopped just short from a fifth-minute tap penalty, Neill plunged over from the resulting ruck.
Green's reliable right boot briefly had Ireland seven points to the good, but Italy mounted a quick response. Opoku Gyamfi drew his side level with a fine score, set up by Pietro Celi's pacy break and offload.
However, a nicely-threaded kick from Christopher Barrett soon caused trouble for Roberto Fasti, under pressure from the chasing Daniel Ryan. The field position and possession led to Ireland's second try, 16 minutes in.
Following a brilliant surge by Diarmaid O'Connell from a scrum, and a close-in penalty, Maguire timed his pick well to burrow in under the posts. Green converted for a 14-7 lead.
Again though, Italy wasted little time in hitting back. The ball squirted free in contact and Italian prop Meroi made the most of it, bursting over from the edge of the Irish 22.
Neill and his team-mates responded to that concession, and the subsequent cooling break, in impressive fashion. A big break had O'Connell, James O'Leary, and Lautsou to the fore.
Despite Green being stopped just short, Ryan used O'Shea's long skip pass to feed Molony who scored out on the right, crashing in between Fasti and Giacomo Falchetto.
The pattern of tit-for-tat scoring was broken as Ireland twice exited well through Ulster Academy talent Green, and Maguire also rumbled forward with great purpose from an overthrown Italian lineout.
Both sides failed to capitalise on maul opportunities deep inside the opposition 22, with Italy threatening twice but UCD clubman Maguire expertly holding up Antony Italo Miranda on the second occasion.
While disappointed to lose second row McGuire, who hobbled off injured, his replacement Seán Walsh from MU Barnhall quickly made an impact with a rip in the tackle.
The Irish backs lifted the tempo, as a free-flowing attack saw Carney breach the Italian 22. Emiliano Mastropasqua was caught offside, allowing Green to extend the lead by three points.
Amid a lengthy rain shower, Ireland had to soak up some early second-half pressure. Mullingar RFC product Luke Murtagh came on to make his first Junior World Championship appearance.
A crucial turnover penalty, won by Neill, broke up the Italian momentum. Once Ireland returned to their opponents' 22, they turned down a kickable shot, but crossing from Ryan denied O'Shea a try in the left corner.
The industrious Irish pack kept chipping away, and while Davide Sette thwarted them twice with lineout steals, number 8 Wilson saw yellow for making head contact with O'Shea.
The 14-man Azzurrini got on the front foot, following some Irish errors in possession, and an overlap on the left was gleefully converted by winger de Novellis.
Mattia Andretti's third successful conversion of the night closed the gap to a single point, yet Ireland's bench then became a factor, with Charlie Keane disrupting an Italian attack for a scrum which was turned into a penalty.
A crisp blindside move off the next lineout saw Ireland reassert themselves. Luke Coffey released Molony from halfway, and his kick infield bounced up perfectly for O'Shea to gather and touch down.
Italy thought they had taken the lead for the first time, just minutes later. Opoku Gyamfi's strength was too much as a bustling ball carrier, only for his try to be disallowed for a forward pass.
A relieved Ireland knuckled down again, winning a scrum penalty before the Connacht-capped Walsh picked off an Andretti pass for his clinching score, converted by Green.
There was still just enough time for Italy to squeeze in their fourth try. Alessandro Ragusi's classy counter attack put fellow replacement Varotto darting over from 35 metres out.
